My spouse is financially dependent on me. In this case, the employer covers 100% of the contribution: I must therefore provide my tax assessment notice, or any other supporting document, to justify their situation as described below.

My spouse is not financially dependent on me, and I wish to enroll them. In this case, the contribution will be debited monthly: I must complete the SEPA direct debit mandate and submit it below with my bank account details.

If there is no response, my spouse will be automatically deregistered on january 1st 2026.

I wish to join the supplementary health insurance plan. The premium will be debited monthly from my bank account I complete the SEPA direct debit mandate and submit it below.
